The Role
As a Home & Community Case Manager, you will play a crucial role in building strong relationships with clients, stakeholders, and team members. You will be involved in the care of clients with a range of health conditions, disabilities, and complex needs. Your role will also include mentoring, developing, and supporting our community team, including Support Workers, to ensure we provide high-quality, client-centered care.
You will conduct in-home assessments, develop care plans, manage budgets, and coordinate services to support individuals in maintaining their independence and achieving their goals. Additionally, you will ensure compliance with NDIS and Aged Care regulations and maintain meticulous documentation to uphold service quality.
The Responsibilities
- Work closely with the team to foster growth, supporting with enquiries and ensuring we continue to build a strong presence in the NDIS sector
- Conduct comprehensive in-home assessments to identify client needs, goals, and required supports
- Develop and implement individualised care plans and rosters of care
- Ensure clear and concise information is provided to Support Workers, ensuring they have mentorship, and training to equip them for their role
- Coordinate and advocate for services related to healthcare, housing, employment, and social integration.
- Ensure compliance with NDIS and Aged Care regulations through accurate record-keeping and reporting.
- Monitor service quality, conduct regular care plan reviews, and implement improvements as needed.
- Manage risks, incidents, and client feedback to continuously enhance service delivery.
- Collaborate with internal teams and external stakeholders to optimise care coordination.
About You
- Experience in case management within the NDIS sector.
- Qualifications in relevant field (e.g. Diploma Community Services, Case Management, Graduate in Allied Health or Nursing, Bachelor of Social Sciences or Social Work)
- Strong understanding of the NDIS framework, care planning, and funding management.
- Excellent communication, customer service, and negotiation skills.
- Ability to work independently and collaboratively within a supportive team environment.
- Strong interpersonal skills with high resilience and problem-solving capabilities.
- Ability to interpret policies, clinical reports, and regulatory requirements.
- Experience working with CRM databases (ideally Salesforce).
- Experience working with individuals with disabilities, mental health challenges, and complex needs.
- Current Driver’s License, First Aid, NDIS Check, and Working with Children Check (WWCC).
