Senior Case Manager - NDIS, Home & Community (Sydney-based)

About the Role
This isn’t a traditional senior management role, it’s a hands-on position focused on sustaining an existing portfolio of 30+ NDIS participants and coordinating a team of 30 Disability Support Workers across NSW during a period of transition.
You’ll operate in a complex, fast-paced environment, so you’ll need to be proactive, self-directed, and outcome-focused. Your priority is to maintain service continuity, ensure compliance, and provide structure and clarity through change.
You won’t have direct reports, but you’ll work closely with frontline staff and support internal coordination and reporting. You’ll also play a key role in mentoring and supporting Support Workers, helping them deliver consistent, high-quality, person-centred care.
Key Responsibilities
- Operate independently in a fast-paced and take full ownership of your portfolio
- Present participant progress, service issues, and funding updates clearly to internal stakeholders
- Conduct in-home assessments to identify each client’s needs, goals, and required supports
- Develop and implement individualised care plans and rosters
- Provide clear, timely instructions to Support Workers, along with ongoing mentorship and guidance
- Coordinate and advocate for services across healthcare, housing, employment, and social connection
- Maintain compliance with NDIS and Aged Care standards through accurate documentation and reporting
- Monitor service quality, review care plans regularly, and implement improvements as needed
- Manage risks, incidents, and client feedback to support safe, high-quality service delivery
- Present participant progress, service issues, and funding updates clearly to internal stakeholders
- Use systems like Salesforce, Excel, and rostering tools to manage caseload and service data
About You
- Experience in case management within the NDIS sector
- Relevant qualifications (e.g. Diploma in Community Services, Allied Health, Nursing, Social Work, or similar)
- Strong understanding of the NDIS framework, care planning, and funding management
- Confident communicator with excellent customer service and negotiation skills
- Able to work independently while contributing to a collaborative team environment
- High resilience and strong problem-solving skills, with the ability to manage competing demands
- Comfortable interpreting policies, clinical reports, and regulatory requirements
- Experience using CRM systems (ideally Salesforce)
- Background in supporting people with disability, mental health, or complex needs
Future Opportunity
This is a stabilisation role — but there’s scope to step into a more senior leadership position after 6 months.
