The Role

As a crucial member of our team, you will play a dual role in both leadership and case management, combining clinical governance oversight with hands-on support.

You will enhance performance across all business sites including high physical support and psycho-social mental health, ensuring compliance with regulatory standards and delivering consistent, quality care to participants with a range of complex health needs.

Responsibilities

• You will ensure high-quality care and drive NDIS-compliant practices and comprehensive health assessments, updating care plans to meet individual physical and mental health needs.
• Support clinical governance and oversee Aged Care and CHSP programs.
• Design and deliver clinical training for support workers and leaders.
• Cultivate trust with clients, families, and stakeholders, offering clinical guidance to achieve positive health outcomes.
• Champion Client-Centered Care. Regularly review care plans, ensuring client preferences, independence, and regulatory compliance are at the forefront of care delivery.

Skills & Experience

• Bachelor of Nursing (or equivalent) with at least 5 years' experience in disability, aged care, or allied health services, including hands-on training.
• Experience in competency-based training and assessment, with the ability to plan, implement, and evaluate service plans.
• Ability to work in a fast-paced, dynamic environment and interpret clinical reports, policies, and processes.
• Strong understanding of disability, dual diagnosis, and complex client needs.
