The Role
As a Case Manager, you play a crucial role in building strong relationships with clients, stakeholders, and team members.
You will be responsible for the oversite and management of clients attending social group programs, accessing the community and also living independently in their home. Focusing on achieving their goals and working closely with their support team. Your involvement in the care and management of clients will include working with a range of needs and disabilities from basic to complex, both in an NDIS and Aged Care setting. You will provide oversight to our support workers through mentoring, developing and supporting them, ensuring we are providing high quality client centered care.
The Case Manager will manage the pipeline of referrals to our services, attend client’s homes to undertake in home assessments, evaluating and mitigating risk and ongoing assessment of their environment and care. They will also ensure annual assessments or updated plan assessments are completed, including goal setting and budget management. This will be done by working closely with their key stakeholders and ensuring clear transparency and individualised care.
The Responsibilities
- Support and drive growth within the portfolio.
Provide support, oversight and mentorship to Support Workers and other internal stakeholders. - Conduct assessments, risk assessments and manage the referral pipeline.
Work with clients to set clear and achievable goals, manage their budget effectively and ensure their care is tailored to meet their individual needs.
Oversite of the Social and Community programs, driving growth and supporting people to achieve goals. - Perform assessments, identifying risk and vulnerability, and review care plans for clients with complex needs.
- Budget and plan management is an essential element of the role, and this will involve working closely with internal and external stakeholders.
- Ensure compliance with Aged Care and NDIS regulations, focusing on meticulous documentation
About You:
- Experience in case management within the NDIS sector is essential.
Exceptional communication skills. - Drivers licence and access to a vehicle.
- Exceptional skills in customer service, negotiation, and rapport-building.
- Strong understanding of the NDIS framework and best practices in client care.
- Excellent time management and organizational skills.
- Ability to work independently and as part of a supportive team.
- Strong interpersonal skills with high resilience.
- Ability to interpret guides, policies, and relevant reports.
- Understanding of disability impacts, dual diagnosis, and complex cases.
Ideally an understanding of Aged Care funding, this is highly desirable.
