Responsibilities:
✨ NDIS Plan Support: Engage with participants to understand their NDIS plans and assist in developing goals aligned with their NDIS Plan.
✨ Service Coordination: Coordinate services on behalf of participants, linking them to relevant providers for comprehensive support.
✨ Community Engagement: Build relationships with informal supports and engage at an organisational and community level.
✨ Capacity Building: Assist in the development of capacity-building strategies, fostering independence among participants.
✨ Ongoing Support: Provide continuous support coordination services to ensure participants' needs are met effectively.
About You
- Minimum of 3 years' experience in a similar role
- Relevant qualifications in Social Work, Community and Mental Health, Drug & Alcohol, Social and Health Sciences are desired but not essential
- Strong understanding of the NDIS and its processes
- Exceptional communication skills
- Own car and driving licence
